PRESCRIPTION ERRORS

Frequently, injuries occur as a result of prescription errors. In fact, the National Academies Institute of Medicine estimates that hospital prescription errors alone cost the American patient population 3.5 billion dollars annually for extra care and injuries arising from the errors.

These errors can occur in a number of ways. Doctors and pharmacies must work hand in hand to ensure that the proper drugs in the proper doses are delivered to the patient. A doctor may incorrectly write a prescription or prescribe a drug that will react with those already being used by the patient. A pharmacy may fail to catch the error, or the pharmacy may fill the prescription incorrectly. If any one of these things occurs, the patient suffers the consequences. Unfortunately this happens with frightening frequency to patients in hospitals, in long-term care, or out.
